refactor: 提取公共格式规范并改进 gitlab-plugin 命令质量

变更内容

• 修复 commit-push-mr 的 allowed-tools 中 git checkout 匹配模式
• 将 Chinese Conventional Commits 格式提取到 references/commit-convention.md，消除两个命令间的重复
• 两个命令新增 git diff 工具权限和近期 commit 历史上下文
• 添加工作区无变更时的终止逻辑
• 移除过度限制的"不要发送其他消息"指令
• commit-push-mr 新增输出 MR URL 步骤

测试计划

• 验证 /gitlab-plugin:commit 正常工作
• 验证 /gitlab-plugin:commit-push-mr 正常工作
• 确认 commit-convention.md 通过动态内联正确加载
